Musha Cay is a private group of 7 islands, but the sandbar is 'public'. You would need to take an excursion or hire someone. Last time we were there (March when we took the tour), the sandbar had 'moved' away from Musha during one of the last tropical storms, so I'm not sure how it looks now.
